I had asked around for what I thought was a really useful patch or program, to have the SPEAKERPHONE default to ON for every call. I found such a program — it works GREAT — but the install is a bit tricky.
I might as well post the whole explanation, in the hope that it helps someone:
The END RESULT of what it dos is to leave the speakerphone in the “on” mode whether you are calling out or receiving a call — the opposite toggle from the way it normally is. I prefer to have it start up that way ; I can set the volume rather low and hear it right close by my ear. But enough of the social implications.

The instructions are rather vague so I’ll go over them.
a) you have to use a registry editor to enter the 3 keys shown on the download page.
( I don’t know why someone clever enough to write a program like this can’t create registry keys… or a CAB file…)
b) You then have to download the .RAR file, and extract the 2 files: (this requires WinRar on the PC)
InTheHand.WindowsMobile.dll -and-
SpeakerPhone.exe
c) you then have to use an explorer program to create the folder (in your phone):
Program FilesSpeakerPhone
d) copy the 2 files from your PC into that directory in the phone.
e) using an explorer program in your phone, navigate to the folder and run the .exe program.
FYI while you are on a call if you press and hold the green/go/yes/call/speaker button for 2 seconds, it now turns the speakerphone OFF.
The NEXT time you make or get a call it will be ON again.
You have to stop the program from the task manager.
I am SO glad I found this. I hope it helps someone else as well.
